If the volunteer is under 18, Sunday Friends requires that you fill out and submit this Parent Permission Form.
In addition, your child must: 1. Read the entire New Volunteer Orientation 2. Fill out and submit the Volunteer Application and 3. Complete a phone interview before s/he will be allowed to participate in a Sunday Friends program.
